* on new 1004 and 1025 forms, the cost approach will be completed. at the above cost. No assignments accepted where fees are to be paid out of HUD-1 closing statement, except for approved Banks, National Lenders and Credit Unions. If such an arrangement is to be made a $50.00 surcharge will be added.
